I would like to thank you for choosing me to be part of your health care team. The process of becoming a new patient can be quite comprehensive. It requires a commitment of time on your part. It can take at least an hour, sometimes longer, to complete the medical history form so set aside a sufficient amount of time in order to fully complete it.
In order to schedule your initial visit, I ask that you please complete the Dr. Cohl Medical Questionnaire and return it to my office 3 days prior to your first visit. This form is interactive, so your able to fill it out via your computer. Please be sure to print out all the other forms and bring them to your initial visit (these forms you must complete by hand. After filling out the Dr. Cohl Medical Questionnaire, you can email it to me at drcohl@drcohl.com. (The Dr. Cohl Medical Questionnaire is the only form I need 3 days before your initial visit)
I would also like you to request records from previous doctors and specialists that you believe would be of value. It is helpful if you send any relevant labs, radiology and pathology reports that you might have in your possession.
My staff will make copies of everything we receive from you and your other doctors. Once this is ready, I'll do a comprehensive review of all the material before you arrive so that I may be prepared to discuss further evaluation and my treatment methods with you.
On the day of your visit, please bring all of your medications and supplements/vitamins. We will look at these together.
